On Your First Visit
We are committed to providing you and your family a passion filled therapy experience with the sole purpose of taking you beyond the place you are to new horizons.
The first visit to our office is designed to help you get acquainted with our therapists and staff. We encourage questions and do our best to always deliver quality care. Thank you for your confidence in Beyond Boundaries.
During your evaluation, you will:
- Meet our office staff to finalize paperwork and answer any questions you may have
- Meet the evaluation therapist
- Schedule a summary meeting with the therapist
- Complete summary meeting and insurance authorizations if necessary
- Schedule therapy sessions
The New patient forms will be emailed to you
